Please complete the following Registration Form to confirm your place in our upcoming Professional Development Trainings. You are able to register for either of the following:

• Module 1 and Module 2 (Early Learning/Lower Primary Educators only). Completion of these trainings enables you to share Yoga Tools in your Lower Primary classroom;
• Module 1 and Module 3 (Upper Primary Educators only). Completion of these trainings enables you to share Yoga Tools in your Upper Primary classroom; or
• All relevant modules offered plus Cert 1 Training (to become a YTFS Program Leader).

We will have a hard copy of this form for you to sign at the Trainings.

Liability Release:
I am aware that the Yoga Tools For Schools Inc. instructors are present to serve me by sharing their knowledge of, and offering me a variety of yoga tools to support my professional development as an educator. I recognise that some of the day's activities may be physically challenging. It is my responsibility to ascertain any medical or other condition that may prevent my participation in any of the day's activity. By my participation, I represent that I am physically fit and have no medical or other conditions that would in any way make it harmful for myself to participate. I agree to take full responsibility and assume full risk for any injury or loss I might suffer while participating in a Yoga Tools For Schools Inc. Training Day. I waive any claim, at any time, for an injury or loss of any sort against the Instructors, and/or assistants from Yoga Tools For Schools Inc. I have carefully read the above release and waiver of liability and fully understand its content.

I understand that there may be photos taken during the training day and that I might be included in these photos. You have my consent to use these photos to support the development of Yoga Tools For Schools Inc.
